What should I bring to my appointment?

Please bring your insurance card, identification card, a referral if required by your insurance carrier, and medications or a list of medications you are presently taking.

Do you call to confirm appointments?

Yes, we have an automated calling system reminding you of your appointment date and time. if you prefer not to be contacted please advise our office. Please allow 24 hours for cancellations.

What is your financial policy?

Payment is expected at the time of service. MasterCard and Visa are accepted for your convenience. Please feel free to contact our billing office to ask any questions prior to your appointment.

What you are allergy shot hours and do I need an appointment time?

No appointment is needed for allergy shots. We are open during regular office hours and during the lunch hour daily. Our last allergy injection is given 30 minutes prior to closing. (Click here for Office Hours)

Can I be allergy tested on my first visit?

Our physicians only order allergy testing after a complete consultation. The testing will then be scheduled after your consultation.

If I need allergy shots can I give myself shots at home?

No, for patient safety we follow national and international guidelines and only give shots in the office under supervision.

How do I get a refill on my medications?

You may fill out our Prescription Refill Form to request a refill on your prescription(s). Alternately, you may call our office to request a refill. Please submit the form/call us 3-5 days before your supply is completed. We ask that you call the pharmacy to confirm your prescription has been called in before calling our office. We do not refill medications after-hours or on weekends. Additionally, there is a $25.00 charge for all prescriptions called in after hours that are not billed to the patient's insurance.
